NAP Tax Credit Paperwork Instructions

Attached are both the Indiana State Tax Form NC-10 and the NC-10 Short Form.  The “Short Form” has recently been introduced to secure donor confidentiality.

Please fill out the TOP TWO sections of each of these forms and return the forms with your check to our office in the envelope provided. 

Please include “NAP” in the memo section of your check.
